Evaluation of renal injury induced by long-term use of adefovir dipivoxil in rats

ABSTRACT  Objective: To observe the effects of adefovir dipivoxil (ADV) treatment for 8 weeks on the kidney in rats.Methods: A rat model of ADV nephropathy was developed by ADV gavage.Thirty rats were divided into 5 groups: the blank control group (or group A),the normal saline (NS) group (or group B),the ADV medication with drug withdrawal at week 0 group (or group C1),the ADV medication with drug withdrawal at week 4 group (or group C2),and the ADV medication with drug withdrawal at week 8 group (or group C3),each consisting of 6 animals.The rats in the group B and the ADV medication groups were respectively given NS and ADV (solved in NS) at a dosage of 1 mg/kg,once a day for 8 weeks in succession.The volume of gavage was 10 ml/kg.Twenty-four hours after last gavage,peripheral blood and urine were collected from the rats of groups A,B and C1.Four and 8 weeks after drug withdrawal,blood and urine were collected from the rats of groups C2 and C3.The levels of serum creatinine (Scr),serum phosphorus,serum calcium,serum β2-microglobulin (β2-MG) and urine β2-MG,and the activity of urinary β-N-acetyl glucosidase (NAG) were detected and analyzed.Then,the kidney was separated to observe histopathological changes in the kidney tissues.Results: There were no significant differences in the levels of Scr,serum phosphorus,serum calcium,serum β2-MG,urine β2-MG,and urine NAG,when comparisons were made between groups A,B and C1 (P>0.05).There was also no statistical significance in the levels of Scr,serum phosphorus,serum calcium,serum β2-MG and urine β2-MG,when comparisons were made between groups C1,C2 and C3 (P>0.05).However, the activity of urine NAG in the animals of group C3 was obviously higher than that of the animals of group C2 (P<0.05). Renal tubular epithelial cells of group C1 displayed mild to moderate swelling and degranulation.There was red blood cell cast in individual tubular cavity,but glomerular structure was intact.There was no obvious renal tubular necrosis or atrophy,and the renal interstitium showed obvious fibrosis and inflammatory cell infiltration.Significant improvement was shown in the injury of renal tubular epithelial cells in group C2,when it was compared with that of group C1.The renal tubular injury in group C3 was significantly improved,when it was compared with that of group C2,but it was not completely recovered 8 weeks after ADV withdrawal.Conclusion: Administration of ADV,at a dosage of 1 mg/(kg·d) for 8 weeks,could induce renal tubular injury in rats.No full recovery was achieved 8 weeks after ADV withdrawal.Clinical routine indicators,such as Scr,β2-MG and so on,could not fully reflect early renal damage induced by prolonged use of ADV.
