Analysis of the pattern of prescription behavior and unreasonable prescriptions

ABSTRACT  Objective: To analyze the medication pattern of doctors in the outpatients and emergency departments,so as to find out the potential links of unreasonable prescriptions and improve the effectiveness of prescription review.Methods: Prescription records of the outpatients and emergency departments from the database of the hospital information system (HIS) were divided by different departments and were randomly chosen for the study.The number of prescriptions,the types and number of drugs were summarized and analyzed by the ABC classification system.Then,irrational medication frequency shown in unreasonable prescriptions was analyzed against prescriptions by physicians from the outpatients and emergency departments.Results: Totally,over 190 000 medication records were collected,including 490 drugs.Included in the analysis were 297 physicians from the emergency department.The number of drugs prescribed by individual doctors was 30-253,with the medium being 156 and the upper and lower quartiles being 111 and 182.In the records of all irrational prescriptions,there were no prescriptions with improper indications,but there were prescriptions with incompatibility or adverse drug interactions.Eight physicians and 10 records were involved in irrational prescriptions,which were viewed as being improper medication and dosage by pharmacists and were then revised by doctors as proposed.With the drug types of 10 records,the drugs in the 5 records were of the most commonly used A-type drugs,and those in the other 5 records were of the frequently used B-type drugs.Conclusion: The number of prescribed drugs by doctors was directly associated with occupational specialty and medical departments.The types and average number of drugs prescribed by physicians were all higher than those by surgeons.The doctors with unreasonable prescription records were consistent with the general pattern of easy to make mistakes due to heavy work load and pressure.Presence of irrational prescriptions was of random low-rate incidence,without a regular pattern.All this indicated that clinical pharmacists should persist in making prescription review to ensure medication safety clinically.
