Establishment and effect of feedback mechanism in the pharmacy intravenous admixture service

ABSTRACT  Objective: To improve the rate of rational use of intravenous drugs and reduce the number of unreasonable medical orders in the pharmacy intravenous admixture service (PIVAS).Methods: From July,2016,the numbers of unreasonable medical orders and rejected medical orders from various wards of the hospital were calculated by the prescription review pharmacists and the feed-back system was thus established.Rejected medical orders prescribed during the period from July 2015 to June 2017 were collected from the PIVAS of Changhai Hospital of the Second Military Medical University,and changes in irrational medical orders 12 months before and after the establishment of the pharmacist intervention feed-back system were compared.Results: From July 2015 to June 2016,pharmacists in the PIVAS handled a total of 4 423 306 infusion bags,and 17 079 bags were rejected,accounting for 0.39%,while from July 2016 to June 2017,4 648 757 bags were handled,and 11 558 bags were rejected,accounting for 0.25%.Statistical significance could be seen,when comparisons were made between them (P<0.01).Conclusion: The feed-back system produced good effect and the number of unreasonable medical orders was considerably reduced,furthermore,the level of pharmaceutical care was obviously enhanced through effective communication between physicians and pharmacists.
